Navigating the maze of medical and social services can be overwhelming. Care managers help clients and family members identify needs and find, apply for, and coordinate services. They can assist to resolve problems with care providers and make adjustments to care plans as situations evolve. Geriatric care managers, who often have nursing or social work backgrounds, specialize in working with older adults and their families.

This non-profit organization is the local provider for this statewide program that provides case management services to frail and low-income older adults to enable them to continue to live safely at home for as long as possible.

This program provides specialized outpatient mental health services to older adults. Services include comprehensive geriatric assessments, individual and group therapy, case management, psychiatric medication and follow up, and housing support.

This program provides in-person case management to connect older adults and dependent adults to services that may enhance their general quality of life including social, recreational and informational activities.

This interdisciplinary clinic serves adults 65 and older as well as their family caregivers. Available services include geriatric medicine, cognitive and dementia testing and care, nutrition consultations, family caregiver services, mobility clinic, patient navigation/case management, and pharmacy.
